Policy Action Form

Policy Action Form - Appendix B must accompany drafts of proposed policies indicating action at each level of the approval process. The Responsible Office shall route the proposed University Policy with the Policy Action Form to the appropriate individuals throughout the process and any relevant information will be noted on the form as it moves through the policy development process.
